To register your team in the Colorado Rapids Adult League please follow the following steps:

Step 2 -
Submit $525 deposit to secure placement into the league. The deposit must accompany the Team Registration form (from step 1).
Cash, check, and all major credit cards are accepted.
Registration DEADLINE: Friday, May 23, 2008
Player Registration:
The league is sanctioned by US CLUB Soccer. For the summer league, each player MUST have a valid 2007/2008 US Club Soccer player card to compete in any division.
Player Card Process – The three items below must be submitted (at least 5 days earlier) before any player can compete in the Colorado Rapids Adult League.
1. Each player must fill out the US CLUB Soccer Adult Player Registration Form (For players that are under 21 years old, please use the US CLUB Soccer Youth Player Registration Form.
2. Each player must submit a color digital photo. A .jpeg file will suffice.
· For the two items above: please send each to llavorato@dsgpark.com . PLEASE also include full name, team name, and contact information (PHONE). If you have problems emailing the form please fax it to 303.727.3701. If you choose to fax the form make sure that your email address is listed so that it can be matched to your photo.
3. The third item is the $26 payment to process the card. Once the league has received your email (that includes a photo and contact information) you will be notified or called so that league staff can collect credit card information to process the player card. Please call 303.727.3521 if you have not received a response in a timely manner.
PLEASE KNOW: Player cards can also be obtained with cash, check or charge at Dick’s Sporting Goods Park. Players that are interested in this method should call Luke Lavorato at 303.727.3521 to set an appointment. Office hours for player cards are generally Monday – Friday, 9 a.m. to 5 p.m. by appointment only.
Once these materials are received they will be submitted to US CLUB Soccer. These cards are then processed and sent back for printing. Because of this delay, ALL MATERIALS MUST BE SUBMITTED BY WEDNESDAY (5 days earlier) if the player intends to play on the upcoming Sunday. No exceptions will be made to this policy.
